ED Triage and Care Transitions Projects Kick Off

BPHC’s projects are beginning to ramp up towards implementation, but at this point many are in different stages of implementation. Among the farthest along are the ED Triage and Care Transitions projects. Implementation for these two projects kicked off on Thursday, October 29th, when the projects’ Implementation Work Group convened for the first time.

Both the ED Triage and Care Transitions Projects will enhance and expand successful systems already in place in the two hospitals in our PPS: St. Barnabas Hospital and Montefiore. The ED Triage project features care navigation services to divert patients from the emergency department to their primary care providers or crisis counseling centers, provided by our partners at Parachute NYC. The Care Transitions project aims to make sure patients being discharged from the hospital do not rebound back to the ED by providing care coordination and management, discharge planning, and, if necessary, a warm handoff with community based organizations.
