To achieve PCMH 2014 recognition, and therefore to implement DSRIP projects, it is vital that our providers access and share comprehensive patient information. The Bronx Regional Health Information Organization (Bronx RHIO) connects our partners through a single health information exchange, consolidating information from everywhere a patient receives care into one complete record. This allows our providers to accurately track service usage over time for individual patients and monitor the health status of larger populations.
We are asking that every provider in our PPS participate in the Bronx RHIO by December, 2017. To begin this process, please submit a completed Application for Participation and a Participation Agreement to the RHIO. These documents and further instructions are available on Bronx RHIO website.
