The Value Based Payment Readiness Subcommittee provides recommendations for sustaining programs that demonstrate value tied to PPS performance in terms of: outcomes, performance improvement, efficiency and cost reduction; effectiveness in transitions of care; impact on reducing avoidable hospital admissions and emergency department visits; and integration of primary care with behavioral health services and social services that address social determinants of health.
The VBP Readiness Subcommittee reports to the BPHC Executive Committee.
