There are two ways to request a vaccine appointment at SBH.

Complete our Vaccination Request Form. A member of our scheduling team will call you to set up an appointment. Please note, we are not scheduling vaccine appointments through calls to SBH.

Join our Vaccine Standby List. SBH ensures that we administer all of our vaccine doses to those who want the COVID-19 vaccine. We often have additional vaccines due to same-day cancellations.
